The form is due by June 15 and can be accessed online at gis.penndot.gov/muniform/login.aspx

The FHWA 536 Form is a report from the Federal Highway Administration on local government highway-related revenues and expenditures. Townships must provide reports on township-owned streets, roads, highways, bridges, and other structures, such as traffic signals and signs. Township administrators are recommended to save the report every few minutes to avoid being logged out of the system.
